What we collect, and what we do with it.

The short version. Two forms on this site collect information, and only if you choose to fill one in. What an employer sends us is passed to EHP Inc., because the program is theirs and the analysis cannot be done without it. We do not sell personal information, we run no advertising or analytics trackers, and we never see the health information of enrolled employees. If you want your information corrected or deleted, write to us and we will do it.

Who this notice covers

This notice describes how Autumndrift LLC, a Michigan limited liability company doing business as Boost Benefits, handles personal information collected through boostbenefits.net and through email correspondence with us. Autumndrift LLC is the controller of that information.

This notice does not cover the Section 125 preventive care program itself. That program is furnished by EHP Inc. and its carriers and provider networks. Once you or your employees enroll, EHP handles the information involved in delivering care under its own notices. Section 7 below sets out where the line falls.

Information you give us

Two forms on this site collect information, and both are optional. Nothing on this site requires you to create an account or provide personal information in order to read it.

The employer request form. Company name, street address, city, state, ZIP, country, the number of full time W-2 employees, whether they are paid hourly or on salary, your payroll provider, any notes you choose to add, and the name, job title, email address, and telephone number of the person submitting the request.

The partner application form. Your name, business name, business type, mailing address, email address, telephone number, the states in which you operate, your licensing and appointment details where applicable, how you intend to work with us, and the acknowledgements recorded at the end of the form.

When you submit either form we also record the date and time of submission, the internet protocol address the submission came from, and the version of the terms displayed to you. We keep that record because it is the evidence that a particular person accepted particular terms on a particular date, and we would rather be able to produce it than rely on memory.

Information collected automatically

Our hosting provider and the content delivery network in front of the site keep server logs in the ordinary course. Those logs record internet protocol addresses, the pages requested, timestamps, referring pages, and browser and device identifiers. We use them to keep the site running and to investigate abuse. We do not use them to build profiles of visitors.

The site loads its typefaces from Google Fonts. Doing so discloses your internet protocol address to Google as a technical necessity of delivering the font files. No cookie is set by that request.

Cookies and similar technologies

We do not run advertising networks, remarketing tags, or third party analytics on this site. There is no Google Analytics tag, no Meta pixel, and no advertising cookie.

Cookies that may be set are functional rather than commercial: session cookies used by the site software when a form is submitted or when an administrator signs in, and cookies set by the hosting and content delivery layer for security and caching. You can block or delete cookies in your browser. Doing so may prevent a form from submitting successfully, but it will not stop you reading the site.

Why we use the information

We use what you give us to answer the question you asked. For an employer request, that means preparing a feasibility analysis and discussing it with you. For a partner application, that means evaluating the application, and, if we proceed, issuing an agreement and arranging your account with EHP.

We also use it to keep records we are expected to keep, to enforce or defend the agreements we enter into, and to comply with law. We do not sell personal information, and we do not share it for cross context behavioral advertising.

Who we share it with

EHP Inc. Information submitted through the employer request form is shared with EHP Inc. so that a program can be modeled and, if you proceed, established. Information submitted through the partner application is shared with EHP Inc. for the purpose of creating and administering your partner account. This is disclosed on both forms before you submit them, and it is not optional: we cannot do the work without it.

Service providers. Our website host, our content delivery network, our form software, and our email provider each process information on our behalf in order to provide their service to us.

Professional advisers. Our own accountants, insurers, and lawyers, where they need it to advise us.

Legal compulsion and transactions. Where we are required to disclose information by law, subpoena, or court order, and in connection with a sale or reorganization of the business, in which case the recipient would be bound by this notice as to information transferred.

We do not share your information with other employers, with other partners, or with anyone else for their own marketing.

Employee health information is not ours

We want this to be unmistakable. Boost Benefits does not collect, receive, or store the health information of enrolled employees. We do not see clinical records, appointment histories, prescriptions, or diagnoses.

The census information an employer sends us in order to obtain an analysis is employment information: counts, wage levels, and pay frequency. It is not medical information. Once a program is established, the member relationship runs between the employee and EHP Inc. and its providers, and the handling of health information sits with them under their own notices and, where it applies, under the Health Insurance Portability and Accountability Act. If you want to know how that information is handled, ask EHP or your benefits administrator for the applicable notice.

How long we keep it

Form submissions and the correspondence that follows them are kept for as long as we are in discussion with you and afterward for as long as we may need them, which for a partner or a client means the life of the relationship plus the period in which a claim could still be brought. Records of acceptance of terms are kept for as long as the agreement they relate to could matter.

Where a submission leads nowhere, we delete or anonymize it on request, and in any event we do not keep inquiry records indefinitely for their own sake.

Your choices and your requests

Michigan has not enacted a comprehensive consumer privacy statute, and several other states have. Rather than sort visitors by where they live, we will honor the following for anyone who asks, whatever their state: tell you what personal information about you we hold, give you a copy, correct it if it is wrong, delete it where we are not required to keep it, and stop sending you anything you did not ask for.

Write to privacy@boostbenefits.net or to the postal address at the foot of this page. We will acknowledge within ten business days and respond within forty five days, and we will tell you if we need longer. We may need to verify that the request is really coming from you before we act on it, and we will not charge you for a reasonable request.

Where a request would require us to delete something we are obliged to keep, or something we need in order to defend a claim, we will say so and explain why rather than simply declining.

Security

The site is served over encrypted connections, form submissions are stored behind authentication, and access is limited to the people who need it. We say plainly that no method of transmission or storage is perfectly secure, and we do not promise otherwise. Please do not send us Social Security numbers, financial account numbers, or clinical information by email or through a form on this site. We do not need them, and we do not want them there.

Children

This site is directed to businesses and to the people who advise them. It is not directed to children, and we do not knowingly collect personal information from anyone under sixteen. If you believe a child has provided information to us, write to us and we will delete it.

Do Not Track

There is no accepted standard for how a website should respond to a browser Do Not Track signal, so this site does not respond to one. Since we run no advertising or analytics trackers, there is little for such a signal to switch off here.

Changes to this notice

If we change how we handle personal information, we will change this notice and change the effective date at the top of it. Where a change is significant, we will say what changed rather than leaving you to compare versions. Continuing to use the site after a change means the revised notice applies.
